Your First Visit We would like to take this opportunity to welcome you to our office. We will do everything possible to make your first visit informative and pleasurable, while providing the best care possible.
On your first visit with us you will be given a consultation worksheet. This worksheet provides us with important information needed to aid us in making an accurate assessment of your situation. If you prefer, when you call to schedule your initial evaluation you can ask to have these forms emailed or faxed to you. Upon completion of your consultation worksheet you will meet Dr. Levine to discuss your history, and have the opportunity to ask questions and voice concerns.
Following your consultation your examination will begin, This will include diagnostic tests that are utilized to properly evaluate your condition.
If your condition requires an x-ray evaluation you will be referred to a board certified imaging center close to our office. After these tests are performed you will be asked to schedule an appointment to review your test results.
At your very next visit we will review your test results. You will be told if you are indeed a chiropractic case. If you are not we will make recommendations as to which health specialty would better suit your needs. If you are accepted for care you will be given treatment recommendations that we feel will help you achieve results. Keep in mind that our goal is to help, educate and to serve you. We will also review with you, if you so choose, key lifestyle factors that affect your health such as diet, exercise and stress management.
If you choose to accept our recommendations then care will begin that same day.
A WORD ABOUT INSURANCE Dr. Levine is a preferred provider in many insurance plans. When calling for your initial appointment It is important to give your insurance information in advance. This way we can verify your coverage and see how much your carrier will contribute toward your care. On plans that we do not participate you may still qualify for out-of-network benefits.

Whether your insurance covers 10 office visits, 20 office visits or no office visits our care recommendations are base on clinical findings and years of experience. We cannot make recommendations based on what an insurance carrier is willing to contribute toward your care.
